San Bernardino County, California (Ontario, CA) – On January 15, 2026, a freak accident resulted in the death of a driver following a collision that sent a metal beam crashing down onto the roadway in Ontario, CA.
The chain-reaction crash occurred shortly after 3:00 a.m. near the interchange with the 15 Freeway. According to the California Highway Patrol (CHP), a Kenworth tractor-trailer traveling westbound was hauling a large excavator on a flatbed trailer. As the rig passed under the interchange in the number three lane, the excavator struck the 15 Freeway overpass above.
The impact dislodged a metal beam from the overpass structure, causing it to fall directly into traffic. The debris landed on a Toyota Camry traveling in the far-left lane. A second vehicle, a Honda Accord in the number two lane, subsequently collided with the fallen beam. Emergency crews transported the victims to Arrowhead Regional Medical Center for treatment of major injuries. CHP Officer Joe Garcia confirmed that one of the drivers, identified only as a woman, was pronounced dead at the hospital.
The driver of the semi-truck, identified as a 50-year-old man from Mira Loma, remained at the scene. Investigators noted that he was cooperative and that neither alcohol nor drugs are suspected to be factors in the crash.
